You want to be on the nose with a sign like this. You want people to immediately understand the "eye test" point — some will get it from the overall sign, others will be helped if you include the words "eye test"

I would keep coming up with as many iterations as you can that use BYU and eye test until you get the catchiest you can get. Nice work!
