We've determined that it's Apple's fault.
No really, the rotation information is in the photo, but iOS strips that information out before uploading, so there's no way for CougarBoard to know which way to rotate it.
The intent was probably to strip out the sensitive data like location, but they just took everything out instead of just removing sensitive information.
