In general, I would say yes. There's certainly several factors affecting that.
If he broke either the radius or the ulna, that would certainly make sense. Also depending on where the fracture is in the elbow, it might be irritating nerves that go down into the wrist. Simply not moving the arm for so long before coming out of a longarm cast can also cause tightness and achiness in that arm. It's typically a combination of the above, but normal to have some discomfort after coming out of a cast.
