There are countries with 50M+ pop where it works.
None of them have perfect systems, but the UK, France, Japan, Germany, Canada, etc., all have much better, cheaper systems than we do, and none of them are small countries.
And when I have to explain to Mexicans and Brazilians why I'd Uber to the hospital instead of calling an ambulance in an emergency, it shows what a joke our health care system is.
