unfortunately, it depends. Those who pay, overpay. Those who get services for
free underpay.
But overall the 3rd party payer system incentives and pays doctor for unnecessary procedures and additional care. Our litigious society incentives doctors to make sure no stone is left unturned regardless of cost so the system basically requires overpayments.
