My oldest son was an underperformer in 7th grade. Admittedly, not to this
degree. But here is my advice, based on a path we took (and some parts didn't take and wish we had).

First, write 7th grade off completely. Hope he passes. If he doesn't, he has to do summer school. But nothing is going to change now, obviously. I wouldn't get worked up over it at this point.

Second, make your own honest assessment of what he is actually capable of in the long term. What are his strengths? what are his shortcomings? What is reasonable and realistic outcome from your perspective (using HS performance as the measuring stick)?

Third, use 8th grade as a progress year, not the year that you expect to declare victory. Be realistic as to your 8th grade expectations. Aim for much better than 7th grade, but I imagine he will still fall well short of what you think he is capable of.

Fourth, starting in 8th grade find a really good tutor or two (assuming you can afford to spend the money). I realize they aren't necessary to do well in school for most kids. But, especially with boys, they tend to indirectly help with organization, accountability and putting together a more systematic approach to studying. They also might spark some interest in a subject matter. Use them weekly. Not just for test prep.

Fifth, target 9th grade as the first successful year. It doesn't count until then anyway. At the point in this process where you feel like your son is on board, share your thoughts with him (i.e. that you don't expect enormous improvements immediately but that you want to help him make measured improvement so that he's ready to hit 9th grade running).

This all assumes your son actually has a desire to do better. Mine did, he was just very young for his grade, immature and unorganized. It took until 9th grade to get him straightened out and semi-focused on doing well in school. He's finishing up his Junior year now and doing very well. It's been a satisfying process for him (and us!).
