I've seen some chapels that at least have a changing table in the men's room
But I've had to change diapers on the floor of a classroom or the seat of my car. Even if there can't be a Father's Room, there can at least be changing tables.

Depends on the layout. I’ve seen them in open areas, or in the handicap stall, and I’ve also seen them
