What you need to find out is if the vaulted ceiling is trussed or stick framed.

If there are vaulted trusses, you should have some access, though it will be tight. If the vaulting is either very narrow trusses or just a framed ceiling, it will be very difficult to run wire without pulling down at least some drywall. This increases the level of complexity and cost fairly substantially.

No, and I think so. Haven't checked. Would be at the apex of a vaulted ceiling.

If there's access, it's very easy to DIY, as others have said. Even if you're adding a new switch, it can be a pain
