If I have surgery when I’m 30 and can pay for it out of pocket, I’d need to keep that receipt for ~30 years if I wanted
to withdraw that amount later in retirement without it being taxed.

Unless your point is to fund the HSA for a few decades with plans to use it all for medical costs when you’re old, in which case I wouldn’t really think of it as a retirement savings vehicle.
