Nobody knows. The doctor claims he can't know prices or it might affect the trustworthiness of his advice. The insurance company claims they can't know because of the complexity of pieces involved. The Insurance Cost Estimator the Insurance company referred me too doesn't know because they need the insurance billing codes used by the doctor, facility, and anesthesiologist for the procedure. Doctor only knows some potential codes, but can't really say which ones they'll use.
