The system we have now is the worst of both worlds.
I would prefer universal catastrophic coverage with a deductible that equals, say 5% of your AGI. Paid for with a payroll tax that gradually phases out at higher incomes. And accompany it with an HSA for everyone with a SSN.
People then pay for medical expenses with tax-free cash, up to their deductible (and can roll into into retirement accounts if it goes unused) so that routine care becomes cheaper over time, and no one has to worry about going bankrupt because they got cancer or whatever.
If you are employed, your employer can simply take what they would have spent on your healthcare and pay you more, either to offset the payroll tax you are now paying or as a contribution to your HSA.
But alas, such a plan please neither the righties or the lefties so it will never happen.
