Company pays all medical and dental premiums and contributes $2,500 to my HSA.
$6,250 out-of-pocket max for my family. $350 eyewear reimbursement per person each year. Life insurance at 3x salary for free. Preventative health/dental is covered 100%. They also give $1,500 health/fitness/hobby reimbursement each year. Happy with the coverage and prices.
