They do let you volunteer yourself to be one.
Of course they won’t pay you more but it’s a great honor. They’ll also let you sit on as many hospital committees as you’ll sign up for though they won’t pay you for that as well… nor do they really want you saying anything during those meetings.. but since it’s a great “honor” they’ll find some schmuck of a doc willing to do so.
