I don't live in Utah, but I would just take a soil sample to your county extension for testing. It's typically very
cheap and they'll tell you exactly what the fungus is and how to treat it.

Could be lots of things. Too much shade, pooling water, bugs, etc. The only way to know for sure if it's fungus

Interesting. We live next to an alfafa field and they are constantly hitting our lawn with water in that area.
