VA notes are the absolute worst. Every EMR is full of useless garbage — nursing notes, HPI, copied labs, endless med lists, and completely normal physical exams. But the VA is extra useless because their notepad style notes don't delineate different sections very well. Have to pay attention a bit more when reviewing.

Mine usually come with a 10-15 page referral and most is just the authorization sheet with a med list. Often doesn't even have a reason for the referral or any notes from the previous doctor. Fun game trying to discover why the patient is there, because they often don't know.
