Clock operator error. Ran the clock after an incomplete pass.

Official ran in to try to stop the first 3rd down play, but no one heard the whistle.

Because the game clock and play clock did not reset how the head referee asked the clock operator to.

It wasn't the operator. The head official said run clock on my whistle.

I have no idea, but everything was done correctly. Maybe the head official didn't notice the game clock kept running.
