Exactly. You are referrencing a rare event in a disease prevalence
that is not 'extremely rare.' Get it. Despite the high prevelance of the trait, you hardly ever hear of significant morbidity and mortality. Why? Because it would have to be extreme heat, extreme exercise, extreme altitude and extreme dehydration all which rarely occur in well-trained and supervised athletes.
