fundamentals break down when you're out of position
which can be caused many things - distrust or inexperience with the play call, poor technique, lack of focus on the moment and last but not least: getting outplayed by an athletically superior opponent
Most of these can be pointed out as a coaching failure or a player failure depending on your preferred bias. But it is not easy to identify the real culprit unless you can watch the practices.
