Not always..
It can be a muffed snap without the ball touching the ground. The snap can be too high, the holder could be bobbling it, etc. I agree that it is rare, but if it's a bad snap and it's not 4th down (and assuming there is enough time on the clock) then the smart thing to do is for the holder to stand and spike it, no harm done.
