Young people get NPPE because they can generate -140mmHg ish pressure when biting an ETT. Other adults are getting it from sustained laryngospasm. These old people are not having laryngospasm while snorkeling.
There are far more likely cardiogenic and noncardiogenic etiologies than NPPE.
