I've told my family that I will either die fishing or at my home. No hospital/care center for me.

If at any point it's determined that I don't have a chance at recovery. I want to be immediately removed from the hospital and spend my final days at home. I can't think of a worse way of going than staying in a hospital/care center just waiting to die in a strange place. Let me die at my home.
