I feel like my family has a pretty good routine when it comes to Disneyland. We get up early to be at the park for rope drop, maximize the fast passes and stroller passes, and try to go with the flow. We rarely wait in long lines (due to the fast passes) and the kids get to do everything they want (and usually multiple times with the stroller pass). My biggest complaint with Disneyland is the locals but if you plan to attend when the SoCal pass is blocked, you can miss a lot of them as well.
It seems that most Disneyworld benefits require guests to stay onsite and has people reserving rides 30-60 days in advance. It seems that it would end up being so structured that it would end up being stressful and not a lot of fun. From my research, I imagine forking over tons of money just to wait in lines (something we don't really do at Disneyland).
