I took a formal logic class with a buddy as LSAT preparation
The professor was pretty lenient about the tests, they weren't in the testing center or even in class. They were take home exams (on your honor not to use outside help taking them). When you were finished all you had to do was drop it in his dropbox outside his office. After the second midterm, this other guy we had sat next to the entire semester didn't show up three class periods in a row. We asked the professor if he had withdrawn from the class (it was pretty small, only about 25 people) and he informed us the guy had cheated on his last exam and would no longer be attending.

I later heard from a TA of the class that apparently the kid had picked up another exam out of the professor's dropbox WHILE THE PROFESSOR WAS IN HIS OFFICE and had copied it word for word before placing them both back in it.

The funniest thing to me is that he could have just used the internet in his dorm to get all the answers he needed. There are plenty of formal logic calculators out there that would have shown you exactly how to do each problem. He chose the hard way of cheating.
