No where in that statement by the prophet does it say there are no consequences if their self governing doesn't work for
each and every person.
I too have seen many examples of students getting more than one chance. One trouble is when what a student did becomes so public the church (or BYU) feels a need to take action.
Another issue is when the student openly displays a hostile attitude with church authorities or honor code employees. Most students don't and so their cases fall way under the radar.
