They usually pay small stuff out of pocket & have supplemental.
Most developing countries have cheap, but mediocre routine stuff, and something high quality for the wealthy and expat communities.
For me, the bigger question would the final declining years. It's one thing to retire to Ecuador in your sixties and seventies, but it's another entirely to be in your eighties having suffered a stroke or heart attack, and far from family or other support groups.

Tell them to start looking at houses in other countries because that's the only way they'll be able to retire
