Having worked in the latter two, insurance companies have done a lot
To reign in prices. Providers left to their own devices being paid on volume alone, this becomes much more expensive.

Insurance companies aren’t perfect by any stretch and go too far at times to reduce claims expenses, but they control costs too and seem more interested now than ever in improving consumer experience and reducing premium and out of pocket expenses.

I would say for profit hospital systems and for profit insurers along with the gov regulators (biggest problem) are responsible for current predicament

Also Utah has some of the best healthcare in the country from a cost standpoint and even from a quality standpoint. Since you referenced IHC
