the doctor then did a "peer to peer" that required less than 5 minutes, and the procedure was approved.  If the insurance company is going to be on the hook for potentially tens of thousands of dollars, and the attending doctor has much better knowledge of the issue at hand, then seems like a simple way to resolve the issue, even if it does seem take a few extra minutes.
