This has been an issue for a long time. The airlines could address it.

If you need an extender for the seat belt, you should have to buy two seats. That should be a question when you book. Then it should default to a two seat option or forced to buy first class.

If you don’t declare you need the extender when booking and you need it, that individual should have to wait for the next flight.
