My best friend lost half his weight in 11 months. He started at the MTC weighing
460 (before weight limits for missionaries). He was 6'1" and had a BMI over 60.

The wife of our branch president at the MTC (we were also MTC comps) worked at a medical office and had some experience with weight loss, so she sat down with him the first week and set him on a diet that consisted of mostly produce, with a serving of whole grain and a bit of dairy and meat for protein and other nutrients (honestly it wasn't far from D&C 89). He dropped 60 lbs by the time we left the MTC and kept going until he reached 220 before our 1 year mark.

We served together about 18 months into the mission and he wanted a challenge so we worked on running. I told him he could do a mile under 7:00 so we trained in the mornings and eventually went to the local HS track where I paced it and he completed the mile in 6:59. After the mission he stayed active playing pick-up ball at the church and even ran (shuffled?) a marathon in his 20s. He ate healthy and maintained it for over a decade.

Sadly he got busy with wife and kids and started gaining weight back in early 30s. He was probably near 300. Then he decided to fix it again and had a heart attack on a treadmill leaving his wife and kids at age 34. It has been devastating for them all.

Whatever else your friend does, spend a lot of that reward money on good doctors and accountability for the rest of his life.

I look forward to regular updates and if he's open to it I'd personally reach out to provide encouragement and support. I've seen this story before and I know it's possible to succeed. I hope to see regular updates.
