The refs missing the fumble call (and again on replay) vs Utah in 2010. His
knee was clearly down! You could also argue for the FG at the end on the game that could have won it anyway, but that kick would have been unnecessary without the blown fumble call.
Riley missing a wide open Hoffman in the end zone at the end of the Notre Dame game in 2012 is close as well.
