Internal medicine. I had UNC medical students working on my teams for 3 years. They are outstanding students, and a significant proportion come from a UNC undergraduate degree. North Carolina SChool of Medicine still admits mostly in-state residents.
That said, it's easy to maintain NC residency while at BYU. I was a CA resident at BYU: voted in CA, kept driver license and car registered in CA, and applied to UC schools as a resident and attended a UC school. As an applicant, my CA residency was never questioned. About half of my UCSD classmates attended undergrad outside CA.
As an active LDS student I'd choose BYU but keep NC residency. UNC is better than the University of Utah school of medicine.
If he doesn't do as well as he hopes, there are other options. Other professional schools are less competitive.
