Once again, this problem is because healthcare wants to act like they aren't
part of commerce.

No other business could get away with adding an exorbitant charge without apprising the customer, "hey, I'm happy to get into that, but it is going to change the scope of services and be an extra charge of $700."

No customer ANYWHERE FOR ANY SERVICE IN THE WORLD would ever pay an extra fee like that after the fact without be apprised of it beforehand.
