You're right on everything except 4 and 5.
To be clear, 4b is good. Just not 4 itself.
Regarding #5, if you're merging well before you need to then you're not a BH but just a bad driver that creates unnecessary traffic. It is a proven issue that merging earlier than necessary creates traffic. The lane is as long as it is for a reason and should be used to it's full capacity.
#4, unless it is a designated turn lane it is 100% appropriate to be in that lane regardless of what others behind you are doing. Again, flow of traffic. It's a lane for a purpose and is not designated for turning but to allow a high volume of cars to pass through.
