already subjective call. #4 can be dealt with presently by the official not allowing the injured player back on the pitch immediately. #1 has merit and I've heard it mentioned many times in circles that have influence, so perhaps that will be a change that is made. #2 has no shot; 3 yards (they use yards, not meters) is a HUGE difference. The purpose of a PK is to award a chance for an egregious foul. Granted, not all PKs-worthy fouls are the same. But making that shift would dramatically change PKs into a guessing game because at that distance a keeper who guesses right will stop the ball much more frequently (he'll have an extra step before he dives laterally or an extra half second to decide to go laterally or stay in place) where presently a well-taken PK will be in even if the keeper guesses correctly.
