Its more the PREDICTABILITY thats the issue IMO. For me, here are 2 plays
Watch the first play of the clip - 3 men line up, 3 men rush straight ahead. The QB, in my opinion, knew exactly what was going to happen before the ball was even snapped and that he would have plenty of time, didnt need to even consider the possibility of a rusher off the edge. This approach worked for BYU well against USC where Tonga was getting pressure up the middle and there was an inexperienced QB who made bad decisions but against a better OL from Washington, better coaching and a better QB who knew what was coming, not so much: https://www.youtube.com/watch?v=nqqMmp-aBUs

Opposite alignment, but again a TD by Washington, see the 4.36 minute mark of the clip. Here we put 6 men on the LOS and do so well before the snap, once again telegraphing what we are going to do. The QB sees what appears to be an obvious blitz (if memory serves me correctly even looking at the sideline and even calling out what protection to use for the obvious blitz but that isnt shown in this clip) THe 5 OL from UW correctly block 5 BYU guys - the guys in the middle and to the QB's blind side. That leaves only 1 BYU guy unaccounted for, the edge rusher who the QB has the easiest view of. As such, the QB knows he only needs to get rid of the ball before the rusher ---- whom he can plainly see ---- gets to him. I dont want to say that it easy BUT it is OBVIOUS in terms of what he needs to do, which he then successfully does.

Defense IMO needs to try to confuse the QB, make the qb move, make them unsure.....if we're not doing that and the QB knows presnap what is going to happen they've got a HUGE advantage. MISC observation: in both plays there was NOTHING done by the DL other than straight ahead bull rushes, making it even easier for the OL to handle them.
