I have 12 employees. I could not care less what they use their PTO for. I just require two weeks notice where possible (obviously you don’t know two weeks in advance if you’re gonna get sick, and if they choose to tell me they’re sick when they’re really not, whatever). I make sure they know I want them take it. Leave it at that.
