I went when I was in Brazil. The dentist said, "I'm going to start drilling...
let me know if if hurts and I will give you a shot to numb it."
It never hurt, I never needed the shot. Filling is still fine almost 30 years later.
That was in Porto Alegre Brazil and the mission had us go to this dentist that was a member of the church. I don't recall her name but she did a good job.
