It's not automatic. I have a friend who married a US citizen like 10 years ago,
and still has not been granted citizenship. I think she had some irregularities in her immigration status before she got married, which has caused issues. The government has told her she has to leave the US and apply for citizenship in her home country, but it might take many years, there's no guarantee she will be able to ever enter the US again, and she doesn't want to be separated from her husband and kids for so long, or force them to emigrate to be with her. US immigration laws and rules are arbitrary, byzantine, and often make little sense.
