Ophthalmology (I know, I know) Residency was great covering call at a smaller regional hospital.
Fellowship was worse than Internship: I was covering two Level 1 1000 bed+(each) hospitals, 3 specialty clinics, and their associated ERs for primary call. During the fall I was on call every other week for 9 weeks and regularly ended up sleeping at the hospital. We also covered all daytime consults, as well, so depending on how that schedule went, there were several days in the week that were essentially 24-hour-48 hour straight call. "Post call" was not a thing that I've heard other residencies/fellowships do, so regardless of the overnight burden still had full surgery/clinic days. The pagers and cell phones never stopped. To top it off, we were paid significantly less than my Intern/resident salary (~$800 take home/week) and we quickly blew through our savings and had to get into some CC debt to finish and move when done. I'm painting a pretty bleak "poor me" picture, but I didn't expect that going in. Certainly made me stronger, as mentioned above.
