Before “reporting her to the state board” for fraud why don’t you just call the office.
Usually in these circumstances insurance is lying 90% of the time. 9% of the time the coders messed up and 1% of the time there’s an honest mistake given the complexity of coding.
No doctor is trying to commit fraud for a forty dollar visit. Most employed physicians don’t even have control of what’s billed in their name.
