Every clip I've ever seen of that happening has been when someone is too busy talking or distracted by something
else that they make a mistake. It seems surprising that it is an instructor that does it, but I think an instructor is the most likely person to do it because they are distracted by the instruction happening. It's like when I miss a turn when driving because my MIL won't shut up when I'm bringing her home from the airport.
