Suggestion - try a cruise for your next vacation
It sounds like many of the problems you have could be solved by the confines/individual freedom of a boat. Pick a few excursions to do together and then let everyone decide how they are going to spend their time on the boat (either together or away from each other).

Maybe his wife needs to feel like she has control to avoid unknowns. Some people don't deal well with unknowns.
