Maybe i'm wrong but one of the core reasons that Obamacare has screwed up the system is that it changed the definition of health insurance.

It used to be up to the consumer and the insurer to work out, to the benefit of both, what coverage was wanted and the price that made it worthwhile for both parties.

You used to be able to buy catastrophic only coverage---no maternity, no chiropractic, no behavioral health--for the single guy that drives truck and only fears cancer and broken back.

That change alone accounts for the majority of the massive run-up in the cost of coverage.
