Yeah it sucks but sometimes the facility is in network, but the doctor on call is not.
Such was the case when my daughter had pneumonia. We took her to the hospital on a Sunday because she was having trouble breathing.
The hospital was in network, but the pediatrician on call was based in Vegas and was only in town to take call for the hospital. Needless to say, he was not in network and we paid significantly more because of it.
