Yes, the parent should name his/her kids as beneficiaries on accounts . . .
and not have his/her spouse as a co-owner or beneficiary. Community property rules could still get in the way, but that can be avoided with a simple partition agreement.

Not a plug for attorneys, but paying an estate planning attorney a couple thousand dollars to think all of this through and set up accounts and legal documents the right way can be well worth it in the long run.
