Pediatric dentist is not at fault, it was an anesthesia related death, the forensic dentist was wrong and is getting sued. case was sent to multiple specialist around the country all came back showing that the forensic dentist was wrong.
Yes it is very common in pediatric dentistry to put crowns on teeth. Longevity purposes and to be able to only treat the tooth one time.

This is actually really bad advice. The teeth will fall out, yes, but that doesn't mean they aren't enormously important
