I would suggest Dr. Michael Harris, who is great and located in Orem. I have a more mixed opinion of Ludmil Manov but he may be available sooner.

Regarding medications, ketamine has worked well and really fast in my family. For the anxiety, gabapentin is less addictive than benzo's and also works quickly and well for some people. This is of course in addition to other SSRI's or SNRI's that take longer to work but can also be really helpful.
