A couple of things that would GUARANTEE cost reductions
1. Allow people to buy insurance across state lines and increase competition around insurance companies.

2. Require medical providers to publish the cost of all medical services and procedures up front. If they give you 3 Advil at the ER they provide you that cost your insurance will be billed before they even administer them.

3. Now that you know the cost for services and procedures determine the average total healthcare cost per individual annually which would include every dollar spent on any insurance covered healthcare cost and then set a payback threshold. For example, let's say the average cost per individual is $2500 a year, you have a clause in your insurance policy that if you spend less than that average amount you receive a rebate of 80% of that delta. So with this scenario you only incur $500 in medical charges you are $2000 below the $2500 average cost you receive a rebate in the form of a check for $1600 (80% of the $2000). Numbers can obviously be adjusted and insurance companies can provide different offers in order to compete for business.

Now you have insurance companies competing for your business but more importantly you have instituted free market principles for insurance customers and the consumer is now shopping for the best deal. Consumers are asking questions and making big financial decisions similar to buying any other product to make sure they get the best deal and everyone in the medical community is now forced to compete for that business.

If you have a serious health issue you're still covered for those major events but no longer are you just letting them perform any service they want with no concern for the actual cost.
