There is a fixed cash price for everything a Doc does and yes it is different for insurance providers.
If a doc is a preferred provider then they accept a smaller payment because that insurance company has promised to refer their clients. In other words its like any other business. "We have lots of client to send your way, will you accept a reduced payment for the privilege of seeing them and working with us?"
