They do, but some only cover part.
Here in Colorado kaiser only covers 50% of the procedure, leaving you with a $10k bill. I found that out the hard way, the day of my initial surgery date (Jan. ‘14), but postponed when I found that out. Long story short, they told me it was covered but was given bad info by them that they acknowledged but didn’t make right. Switched to United and they covered 80%, leaving me with about $3k of expenses.

100% totally worth doing the surgery.
